    Modelling the distribution of health related quality of life of advancedmelanoma patients in a longitudinal multi-centre clinical trial using M-quantile random effects regression

    Health-related quality of life assessment is important in the clinical evaluation of patients with metastatic disease that may offer useful information in understanding the clinical effectiveness of a treatment. To assess if a set of explicative variables impacts on the health-related quality of life, regression models are routinely adopted. However, the interest of researchers may be focussed on modelling other parts (e.g. quantiles) of this conditional distribution. In this paper, we present an approach based on quantile and M-quantile regression to achieve this goal. We applied the methodologies to a prospective, randomized, multi-centre clinical trial. In order to take into account the hierarchical nature of the data we extended the M-quantile regression model to a three-level random effects specification and estimated it by maximum likelihood

    Peri-urban agriculture is a pivotal issue in the debate on sustainable management of land in metropolitan regions worldwide. Multiple socioeconomic and environmental solutions introduced by new models of peri-urban agriculture are playing an important role in planning and management of fringe land. The recent development of peri-urban agriculture in Southern European cities was supposed to reflect latent, crisis-driven processes of 'coming back to land': new land has been extensively cultivated, and new relations have been created between farmers, communities and territories within peri-urban areas. This study describes some relevant experiences of peri-urban farming in 6 metropolitan regions (Lisbon, Barcelona, Marseille, Rome, Athens, Istanbul) representative of different socioeconomic contexts in Southern Europe, outlining strengths and weaknesses in the use of fringe land for cropping, and evidencing relevant implications for urban sustainability

    An empirical flood fatality model for Italy using random forest algorithm

    Due to an increasing occurrence of natural hazards, such as floods, a significant number of lives are lost each year worldwide. The risk of experiencing catastrophic losses from flooding is exacerbated due to the changing climate, and the increasing anthropogenic activities. Consequently, predicting the conditions leading to fatalities is crucial in the assessment of flood risk. However, the existing modeling capabilities in this field, are limited, emphasizing the critical need for the development of such tools. Here, we show that the occurrence of flood fatalities can be estimated using a random forest (RF) algorithm applied to nine explanatory variables characterizing each fatality. Furthermore, by converting the RF model outcomes into a user-friendly tool, it is possible to predict the probability of the occurrence of flood-related fatalities, based on variables describing hazard intensity and the environmental and sociodemographic conditions that contribute to such events. Our results represent an initial attempt towards a predictive model of flood fatalities in the Italian context. They reveal the key factors that together influence flood fatalities, enabling the prediction of such occurrences. These findings can serve as a foundational framework for quantitatively assessing the risk to the population from such events and as a valuable resource for identifying strategies to mitigate flood risk

    Governance changes in peri-urban farmland protection following decentralisation: A comparison between Montpellier (France) and Rome (Italy)

    Many countries have implemented laws and planning instruments to preserve farmland on the urban fringe. This paper aims at a better understanding of the governance changes in peri-urban farmland protection following decentralisation processes in France and Italy. We compare the implementation of farmland protection instruments in the two city regions of Montpellier and Rome. From a governance perspective, we highlight the practical issues of effectiveness and social acceptability arising from power devolution, different forms of governance, and the potential conflicts when planning control shifts to lower-than-regional bodies. Our analysis is based on qualitative methods. Primary data were collected through document analysis, participant observation and in-depth interviews aimed at understanding local stakeholdersâ\u80\u99 practices and points of view on access to farmland, housing and building rights. We find that around Rome and Montpellier, decentralisation has produced multiple decision-making authorities and increased the complexity of procedures. Despite more regulatory constraints in agricultural areas, farmland conversion has persisted. However, decentralisation processes have also changed ways of governing and favoured local alternative initiatives for farmland protection and farming development on the urban fringe. New modes of governance involve public local authorities, farmersâ\u80\u99 representative bodies (Montpellier) and civil society organisations (Rome). In both cities, they have a positive but limited impact on the effectiveness of farmland protection instruments. Their social acceptability varies, depending on who is really included in the participation process

    Economic landslide susceptibility under a socio-economic perspective: an application to Umbria Region (Central Italy)

    AbstractAn accurate understanding of physical and socio-economic effects of landslides is fundamental to develop more refined risk management, mitigation strategies and land use policies. In this paper we develop a measure to consider the interconnection between physical and economic exposure, i.e. what we call the economic landslide susceptibility, namely the probability of landslide occurrence in an area weighted for its socio-economic exposure considering the real-estate market values. The economic landslide susceptibility is estimated through a method designed for large areas that we applied to the Umbria Region (Central Italy). The method makes use of landslide susceptibility maps and a real estate market value map for any given area under analysis. The innovative concept of economic landslide susceptibility (that is de facto an ex ante landslide cost assessment) may be interpreted as the potential loss that an area might suffer in terms of its propensity for landslides. Useful applications of the proposed method lie in a better territorial management and in the land use planning

    Acquiring vulnerability indicators to geo-hydrological hazards: An example of mobile phone-based data collection

    Abstract Geo-hydrological risk reduction is a key issue for local governments in Italy. In this context, a collaboration was undertaken between multiple actors in the La Spezia municipality aimed at: (i) monitoring building characteristics, using specific and valuable indicators, and (ii) increasing the knowledge of geo-hydrological hazards across residents and local land planners (iii) implementing local emergency civil protection plan. An extensive mobile data collection was carried out through apps specifically developed for Android and IOS mobile devices. The digital forms were differentiated on the basis of the potential hazard: one of 46 fields and one of 125 fields designed for buildings respectively located in flood prone areas and in medium to very high landslide susceptibility areas. The digital version of the forms was designed using the Open Data Kit (ODK) and GISCloud client-server approach. All the collected data, including geospatial locations and images, were automatically sent to a central server, stored and organized in a database. Geospatial data-analysis and maps resulted useful in evaluating possible impacts to exposed buildings to potential geo-hydrological processes. The proposed public participation method for data-gathering increased the knowledge across residents providing a better understanding of the urban systems, of the buildings condition and their relation respect to the geo-hydrological risk. The method can be considered as part of the decision support systems for civil protection purposes to better planning geo-hydrological mitigation measures. The application of mobile technology for data collection can be effectively used when local government resources are limited

    Role of enhanced glomerular synthesis of thromboxane A2 in progressive kidney disease. Normotensive rats of the Milan strain (MNS) spontaneously develop focal glomerulosclerosis. In order to explore the contribution of glomerular thromboxane (TX) A2 synthesis to the development of the disease, we have characterized the time course of renal functional and biochemical changes, and their modification by long-term treatment with a TX-synthase inhibitor. Oral administration (150mg · kg-1 from 1 to 14 months of age) of FCE 22178 suppressed enhanced glomerular TXB2 production at all experimental times (mean inhibition 80%) and proteinuria (varying between 27.1 and 73.0%) while preserving renal blood flow and glomerular filtration rate. These effects of TX-synthase inhibition were seen in the absence of any statistically significant changes in systemic blood pressure. Moreover, FCE 22178 had no antihypertensive effects in hypertensive rats of the Milan strain (MHS) nor in spontaneously hypertensive rats (SHR). Treatment also prevented the age-related hypoalbuminemia and hyperlipidemia observed in control MNS and significantly (P < 0.01) reduced glomerular histologic damage, as demonstrated by light microscopy studies and measurement of sclerotic area. We conclude that: 1) MNS rats provide an animal model of long-lasting proteinuria characterized by an age-related increase in glomerular TXB2 production paralleled by progressive loss of renal structural integrity and function and by a secondary dyslipidemia; 2) pharmacological inhibition of glomerular TX-synthase attenuates the structural as well as the functional expression of kidney disease, without a primary effect on systemic blood pressure. These data are suggestive of an important modulating role of TXA2 in the progression of MNS renal disease

    Flood Fatalities in Europe, 1980-2018: Variability, Features, and Lessons to Learn

    Floods are still a significant threat to people, despite of the considerable developments in forecasting, management, defensive, and rescue works. In the near future, climate and societal changes as both urbanization of flood prone areas and individual dangerous behaviors could increase flood fatalities. This paper analyzes flood mortality in eight countries using a 39-year database (1980-2018) named EUFF (EUropean Flood Fatalities), which was built using documentary sources. The narratives of fatalities were investigated and standardized in the database reporting the details of the events. The entire dataset shows a stable trend on flood fatalities, despite the existence of individual increasing (Greece, Italy, and South France) and decreasing (Turkey and Catalonia) trends. The 2466 fatalities were mainly males, aged between 30-49 years and the majority of them happened outdoor. Most often people were dragged by water/mud when travelling by motor vehicles. Some cases of hazardous behaviors, such as fording rivers, were also detected. The primary cause of death was drowning, followed by heart attack. This work contributes to understand the human-flood interaction that caused fatalities. The changes in society's vulnerability highlighted throughout this study contribute to manage future risks, to improve people protection actions, and to reduce risk behaviors
